2 Contact Forms with Logic/Conditional Fields

This is an example of the “conditional fields” feature applied to a basic contact form. If you answer ‘Yes’ to the question ‘Are you a representative of our company?’ then 2 more fields will show up and they must be filled. If you then select ‘No’, those 2 fields will get hidden and obviously not required.


The following form shows how a field is shown once you make a selection that is available from the second dependant drop-down once you selected an option from the first one.

e.g. choose “Another option” from “Drop-Down 1”, then choose “Dummy Option 2” and “Custom Input for Dummy Option 2” will show up and it will be required to submit the form.
